How to read it
Tirzepatide has a licensed titration ladder, which makes it the most clearly defined schedule on this site: 2.5 mg weekly for four weeks, then 5 mg, then steps of 2.5 mg every four weeks up to a maximum of 15 mg. The 2.5 mg stage is a starting dose intended to build tolerance, not a maintenance dose.
The licensed product is a pre-filled pen at a fixed concentration. If you reconstitute a vial yourself you choose the concentration, so unit counts quoted for a pen will not match your own.
A titration ladder is a maximum pace, not a target. Holding at a lower dose for longer is a normal outcome, not a failure.

- What is the standard tirzepatide titration schedule?
- 2.5 mg once weekly for four weeks, then 5 mg for four weeks, then increases of 2.5 mg every four weeks as tolerated, up to a maximum of 15 mg weekly.
- How many units is each step?
- It depends on your concentration. In a 30 mg vial with 3 mL of water, which is 10 mg/mL, the ladder runs 25, 50, 75, 100, 125 and 150 units. The last two exceed a 1 mL syringe, so use less water if you are going high.
- Can I skip the 2.5 mg stage?
- The starting dose exists to build tolerance to gastrointestinal side effects. Skipping it is the usual reason people find the drug intolerable and stop.
